Kentucky DMV (Vehicle) Bill of Sale
The Kentucky Department of Motor Vehicles (DMV) provides a bill of sale form for residents to transact vehicles with one another specifically for private or dealer sales. The form should provide information about all the parties involved in the transaction including the VIN number and sales price.
- If the vehicle has a lien on it, the new owner may request a Lien Affidavit

- Year
- Make
- Model
- Color
- VIN
- State Title Number (#)
- Engine Size
- Odometer Reading | Miles or Kilometers
- Damage Disclosure and Warranty
- Payment/Purchase Price
- Signature of Buyer and Seller – Must be completed with a Notary Public present
